• If your child is already receiving services from us, please contact the admissions department at 781-895-3200. 
• If your child is new to Milestones Please download our application and email, fax, or send it back.  (put it here)
• Once we review your child’s application and get back to you within 72 hours to let you know if your child is an appropriate match and when your child can start. 

We have a rolling admissions progress and children can join at any point during the year.  Spots are available on a first come basis. 

Application

Please download our application and send it back to us via email, fax, or through the mail.  Our admission’s department will get back to you within a few days after receiving it.  If you already receive services from Milestones, please give our admissions department a call to discuss the group 781-895-3200.
